These policies were created to give both of us the best experience possible.

For your first bodywork session please allow an additional 15-20 minutes for assessment and paperwork. If you download the health history form and fill it out before you come in, that can save some time.

• I will be prompt for my appointments and ask you to be as well.
• If your practitioner is late your next appointment for the same service is free.
• If you are late, it will take away from the time I have reserved for you.
• I provide draping for clients, and safe and non-sexual massage.
• Please maintain your personal hygiene.

I accept cash, checks, and MasterCard and Visa.

Payment is due at the time of service.

I also take Premera, Lifewise, Cigna, and Aetna insurance. For these I will need you to send me your prescription which has the doctor's name, diagnosis codes, the frequency and duration of treatments. I need to get approval from the insurance company before we can begin.

I require that you give 24 hours notice prior to canceling an appointment. If less than 24 hours is given, you will be charged the full rate of the massage. Your appointment time is reserved for you and you alone.

If you have a contagious condition, please contact me as soon as possible and reschedule your appointment.
